Deadline for all awards not otherwise stated, is Feb 1. Please check each award for earlier date.

Please adhere to the deadlines stated here - not the deadlines in ADA's instructions.

• Duplicate the application forms as needed
• Follow all guidelines to the letter!
• Submit ONE original and FOUR copies of the packet for each nomination
• All packets should be neatly and professionally presented.
  

Nomination packets should include the following:

• A typed data sheet (additional sheets can be included) signed by the applicant.
• A letter of nomination from the sponsor/district association.
• A resume and letters of support (3 maximum) may be included (highly recommended).

Please send 1 original plus 4 copies of the required forms, postmarked by February 1 (or earlier date if stated), to:
